At Abby Care, we’re on an ambitious mission to build a new care system for the most vulnerable families in America.
Today, we’re excited to announce new partners in our mission to build the future of care at home. Abby Care has closed our latest round, led by Transformation Capital, with participation from CVS Health Ventures, Frist Cressey Ventures, and Pivotal Ventures, alongside our existing partners Khosla Ventures, Thrive Capital, and Sequoia Capital.
This partnership will enable us to bring our model to more families, deepen our work with health plans, continue building the technology and clinical infrastructure that makes high-quality, accountable care at home possible, and be the best stewards of public dollars.
Turning America’s 60 Million Family Caregivers into the Backbone of a New Care System
Today, our healthcare system is largely reactive, and traditional home care models haven’t kept pace. Information is scattered across legacy systems and paper. Patients wait months for care. Clinicians can’t see what’s happening in the home, so changes in condition go unnoticed until they become emergencies.
But meaningful, proactive care requires understanding what happens beyond the doctor’s office or hospital, and integrating clinical care with the day-to-day realities of living with a complex health condition.
Family caregivers are uniquely positioned to provide that understanding. They know their loved ones’ routines, observe changes in their condition, and are often the first to notice when something isn't right.
At Abby Care, we build on that existing relationship by equipping family caregivers with professional training, certification, and ongoing support from dedicated RN care managers. Our platform gives care teams real-time visibility into care delivered in the home, helping ensure families receive the support they need while enabling health plans to verify that care is being provided.
By bringing together family knowledge, clinical oversight, and technology, we aim to make care more proactive and preventative; helping people remain safely at home while reducing avoidable emergency room visits, hospitalizations, and costly institutional care.
As our Founder and CEO, Havi Nguyen, explains:
“Abby Care was built around the thesis: when you leverage the person who already knows a family member best and is consistently there for them, and combine that with clinical oversight, AI, and real-time visibility, you can deliver care that is more proactive and preventative than other traditional care models. This is how you can build a better care system for millions of families that need care."
Scaling Our Impact for Families and Health Plans
Over the last two years, Abby Care has grown its patient population more than fivefold. Today, we serve thousands of families across eight states, with additional markets expected to launch soon.
Over the last year, we’ve grown our footprint across multiple states and helped families provide millions of hours of care to their loved ones.
That growth reflects a growing need for a different approach to home-based care; one that supports family caregivers, helps health plans manage members with long-term or complex needs, and makes quality and accountability central to the care experience.
This latest round of investment in Abby Care will support three key priorities:
Together, these investments will help us scale responsibly while continuing to put families and quality of care at the center of our work.
The Road Ahead for Care at Home
These latest partnerships mark an exciting next chapter for Abby Care, but the mission that brought us here remains the same: empowering families to access high-quality care at home, with the support and resources they deserve.
We’re grateful to our investors, health plan partners, clinical teams, caregivers, and the families who trust us to be part of their care journey. Their partnership and commitment make it possible for us to keep building a better model for care at home.
Success means helping many more families access preventive care, easing one of healthcare’s largest workforce shortages, and helping reduce the total cost of care by keeping members safely at home instead of in institutional care.
We look forward to bringing Abby Care to more communities and continuing to demonstrate what’s possible when family caregiving is supported by clinical expertise, thoughtful technology, and a shared commitment to better care.
The future of healthcare starts at home. Time to keep building.
